Uptown Dental treats your entire family! Your first visit to Uptown Dental is all about you!

Ready to come see us? Patients are seen in our dentist office by appointment only. We make every effort to seat you at your appointed time. Therefore, we ask that you extend the same courtesy to us by arriving on time. At Uptown Dental, we strive to create a schedule that meets the dental needs of all our patients. Request an appointment time that works for your schedule.

Scheduled your first appointment? We can’t wait for your visit! We want to make your experience as convenient and comfortable as possible. To save you time, we provide new patient forms that you can fill out before your visit. Just download, complete, print and bring them with you to your appointment.

Fee and Payment Policy
We offer several financing options for your convenience:
Cash or check at the time of service. Payment plans and prepayment discounts for Visa, MasterCard, and Discover payments. If you need extensive treatment, we offer payment plans through CareCredit®. If you have insurance, we will verify your benefits and coverage to determine your portion as accurately as possible. Payment is due when services are rendered. Please ask if you have any questions concerning the costs involved in your care.

Insurance
If you have dental insurance, we will help determine your available coverage. We require that you assign your benefits to us. Any balance remaining after your insurance company has paid us is your responsibility. Please refer to our Fees and Payment policy section above.
Professional care is provided to you, our patient, and not to the insurance company. The insurance company is responsible to the patient and the patient is responsible to the doctor. We will help in filing your claim and handling the insurance questions.
